Reason for Trip (Free Form Entry)
If Athletics, list practice, game, tournament. All others list Club Activity, Extra Curr Activity or Field Trip.
- If the trip is a curricular field trip:
– Attach a field trip rationale form
– Enter the lead teacher as contact
– List chaperones in Notes field
– Identify substitute coverage needed by block
– Identify special needs such as Nurse, AED, EpiPen or glucagon below Notes
