All drivers are required to have a physical examination prior to the expiration date indicated on your current medical exam certificate. This certificate must be carried in your wallet at all times.

Your Examination Options:

Option 1: District-Approved Center: You may use the current District-approved examination center (see link below). The cost of this exam will be covered by the District.

Option 2: Personal Physician: You may utilize your own physician,* provided they are a CDL FMCSA Registered Medical Examiner*. This option is at the driver’s own expense.

Required Action Steps:

  • Obtain Forms First: Drivers must obtain all necessary medical forms from the transportation office before going to the facility.
  • Complete Exam: Complete the physical examination before the expiration date.
  • Submit Documentation: Submit the updated medical certification to the transportation office immediately after the exam.

Upon successful completion of your physical examination and receipt of clearance:

Driver Action Required:

  • You will be issued the Medical Examiners Certificate (ME Card) and the completed Physical Examination Certificate (long form).
  • You must hand-deliver both documents to the transportation office immediately. Timely submission is required to maintain your CDL status.

Office Action:

To ensure efficient and timely processing, the office personnel will then submit your completed Physical Examination Certificate (the long form) to the New Jersey Motor Vehicle Commission (NJ MVC) in Trenton, NJ, via email or fax.
